US robotic surgery shifts from capital equipment to AI data businesses as Intuitive and Medtronic reshape institutional investment

Surgical robots have crossed a threshold. What was once a capital equipment story is now something more interesting: institutional investors are reclassifying the leading US platforms as AI data businesses, and the numbers are starting to agree with them.
Intuitive Surgical's Q1 2026 results made the case plainly. Revenue hit $2.77 billion, up 23 percent year-over-year. Recurring income from instruments, accessories, and services accounted for 86 percent of that total. Management raised its full-year procedure growth forecast to 13.5–15.5%, driven not by new system sales but by deeper use of an installed base that now spans 11,395 da Vinci units worldwide.
The mechanism behind that growth is a data flywheel unique to robotic platforms. Every procedure generates video, kinematic, and clinical outcome data that feeds AI training tools, intraoperative guidance, and post-case analytics. More cases produce richer data. Richer data improves performance. Better performance pulls in more cases.
Medtronic entered the US market in February 2026, completing its first commercial Hugo system cases at Cleveland Clinic. Aimed at community and mid-sized hospitals priced out of existing robotic programs, Hugo connects to the Touch Surgery platform for AI analytics and remote tele-proctoring. A second major data engine is now running in American operating rooms.
Risks are real. Hugo's rollout remains early-stage. Reimbursement for new procedure types moves slowly. GLP-1 drugs have trimmed US bariatric robotic volumes by roughly 10%. None of that is trivial.
Still, from a long-term capital perspective, the framing has shifted. Between 2022 and 2025, surgical robotics attracted $2.4 billion across 42 global deals. In 2026, analysts are no longer debating whether the sector merits investment. They are asking how to price a recurring-revenue, AI-enabled surgical infrastructure business with a decade-long head start. That is a different conversation entirely.
